The actual physiology of Bicarb is often inadequately understood, and the need for ventilation as an adjunct is underused. Many simply consider the dialysis a magical tool that removes acidosis. And by the way, I must say that I completely agree with your frustation with the abusurdity of the current state of RCT´s and long term outcome in ICU an dperiop. trials. The neglect of physiology has prevented any major advance in outcome for years.

Funny how the bicarbonate hate is not extended to dialysis solutions. I suppose some sort of transubstantiation occurs as it crosses the filter membrane, preventing all the ghastly harms.
